This document is a notarized statement from A. W. Cockrell, Jr. notifying the Pension Bureau of the loss of the
original pension certificate. He knew both Thomas and Rachel Wallace for an extended period of years and of his
death in Jacksonville. Each quarter she had to give an affidavit for the pension check. The first firm was A. W.
Cockrell & Son formed in 1883 and in 1907 the successor firm of Cockrell & Cockrell. It was believed to be
destroyed.
